This is how you get started: 

  1. Download the Proviso Township Neighborhood Stabilization Program brochure for information about program requirements. NSP Application (English) NSP Application (Spanish)
  2. Complete a Proviso Township NSP Application form. Accepting applications from September 1st through September 30th, 2010.
  3. Return Application to Ana Fremont or Grazia Cortes at The Resurrection Project offices located at Our Lady of Mt. Carmel Community Center, 1115 N 23rd Avenue Melrose Park, IL 60160. Or you can contact the offices at 708-345-3632 extension 310 or extension 308 for more detalils.
